A commonly used and simple one is the Mosteller formula.

BSA = (W*H/3600)^(1/2) = 0.016667*W^(0.5)*H^(0.5)

or even simpler:

BSA = (W*H)^(1/2) / 60

or if Ht is height in m:

BSA = (W*Ht)^(1/2) / 6

BSA is in m^2, W is mass in kg, H is height in cm, and Ht is height in m.
